Brief Abstract: This rigorous systematic review and meta-analysis investigated whether older adults experience significant alterations in taste perception compared to younger adults. Analyzing 18 observational studies (including 2,728 individuals), the research demonstrated that elderly individuals require significantly higher concentrations to distinguish sweet, salty, and umami flavors. Interestingly, sour and bitter tastes did not show statistical differences between the age groups. From a clinical standpoint, this decline in taste acuity can lead older individuals to find high concentrations of sugars and salts more pleasant, potentially driving dietary changes. Brief Abstract: This comprehensive state-of-the-art review explores the critical factors involved in the etiopathogenesis of root caries, focusing on how microbial collagenases breakdown the organic dentinal matrix. While conventional caries protocols focus primarily on biofilm-driven demineralization, recent molecular advancements reveal that proteolytic and periodontitis-associated bacteria (such as Porphyromonas) enrich subgingival lesions, overexpressing genes that encode bacterial collagenases—specifically from the U32 peptidase family. The study highlights that while the first stage of mineral loss can be reversed, the subsequent degradation of the exposed type I collagen matrix is irreversible. Proving the definitive clinical activity of these microbial enzymes remains a significant challenge. Brief Abstract: This comprehensive review and innovative in silico analysis maps the presence, methodology, and overlooked clinical significance of the human oral archaeome. While the bacteriome dominates dental research , low-abundance archaeal signatures—predominantly methanogens—are consistently present in subgingival plaques, infected root canals, and carious biofilms, showing strong correlations with severe periodontitis and painful endodontic infections. The study explores their syntrophic cross-feeding partnerships with proteolytic bacteria, suggesting they act as key components in oral dysbiosis. A critical highlight of the research is a rigorous in silico primer coverage analysis testing 44 primer pairs from existing literature against current databases. The authors revealed that several standard primers used in oral research possess 0% overall domain coverage, significantly underestimating real archaeal diversity. Brief Abstract:This high-impact study successfully engineered and optimized a robust, clinically relevant in vitro dysbiotic biofilm model to mimic root caries development and test innovative microbial modulation strategies. Using a multi-volunteer saliva inoculum exposed to cariogenic sucrose cycles, the research evaluated two distinct pipelines: a “pre-treatment strategy” on bovine root slabs and a “post-treatment strategy” using a modified Calgary biofilm device coated with collagen and hydroxyapatite to screen targeted natural compounds. Through a comprehensive multi-methodological approach—including metatranscriptomic sequencing, confocal microscopy, scanning electron microscopy (SEM), and micro-computed tomography —the authors demonstrated significant shifts in the biofilm’s metabolic profile. The tested natural interventions effectively modulated key cariogenic pathways and suppressed microbial collagenase activity, halting mineral loss and safeguarding the organic dentinal matrix. The research meticulously screened existing literature to determine which biomodification agents perform best in reducing both root caries lesion depth and the release of hydroxyproline—a definitive amino acid marker of collagen matrix breakdown. By executing a comprehensive network meta-analysis of laboratory data, the study establishes a clear statistical hierarchy among the tested agents (such as proanthocyanidins, glutaraldehyde, and carbodiimide). Brief Abstract: This robust cross-sectional study evaluated 170 adult and elderly individuals (with and without Type 2 Diabetes Mellitus) to map how chronic oral conditions correlate with inadequate macronutrient intake and systemic health risks. The clinical data revealed that compromised oral parameters—such as significant tooth loss and the use of unstable dental prostheses—strongly impact dietary choices, shifting consumption away from essential fibers and proteins toward easily masticated, lower-quality carbohydrates and fats. Structurally, these dietary changes showed definitive associations with increased waist circumference risk, compromised weight status, and aggravated glycemic profiles in diabetic patients. Brief Abstract: This highly significant cross-sectional study investigated the definitive relationship between aging and salivary gland dynamics, establishing a precise annual rate of decline for both resting salivary flow (RSF) and stimulated salivary flow (SSF) while adjusting for critical confounding factors such as type 2 diabetes (T2D), polypharmacy, and dietary fiber intake. Analyzing clinical sialometry data from 166 individuals, the research revealed a striking prevalence of hyposalivation within the cohort. Through unadjusted and adjusted robust linear regression models, the study mathematically demonstrated that salivary output diminishes linearly with each advancing year of age. This reduction is heavily exacerbated by systemic chronic diseases and the cumulative burden of xerostogenic medications. By pooling quantitative sialometry data from multiple global studies through a restricted maximum likelihood random-effects model, the research establishes definitive, updated normative averages for both unstimulated salivary flow (USF) and stimulated salivary flow (SSF) in healthy individuals aged 18 to 60. Furthermore, the study performs precise pairwise meta-analyses directly comparing healthy cohorts against patients suffering from hyposalivation-associated conditions (HACs). Brief Abstract: This high-level bioinformatic study mapped and compared the prevalence, diversity, and activity of the oral archaeome in carious and caries-free environments. Recognizing the traditional challenges in cultivating and isolating archaea, the research utilized a tailored shotgun metagenomic and metatranscriptomic analysis pipeline to screen large, publicly available datasets of human saliva and dental plaque. Reads were mapped against a custom-curated archaeal genome database using highly stringent filtering criteria (>97% similarity and 90% query coverage). The computational analysis revealed distinct patterns in archaeal prevalence and gene expression profiles between the cohorts. While certain methanogenic species showed a clear correlation with advanced carious lesions—likely driven by metabolic syntrophy with acidogenic bacteria—the study also identified specific archaeal signatures in healthy, caries-free samples. The clinical trial mapped structural and chemical parameters including salivary flow rate, pH, buffering capacity, viscoelasticity, and exact micro-concentrations of glucose, calcium, phosphate, and urea. Utilizing adjusted robust Poisson regression models, the research demonstrated that specific changes in salivary ion concentrations and physical properties significantly correlate with caries activity. Brief Abstract: This innovative study presents a rigorous exploratory interim analysis of a randomized clinical trial investigating the therapeutic efficacy of low-level laser therapy (LLLT) and transcutaneous electrical nerve stimulation (TENS) targeting major salivary glands in type 2 diabetes mellitus (T2DM) patients suffering from salivary hypofunction. Moving beyond traditional sialometry assessments, the research pioneered the evaluation of how physical neurostimulation modalities actively modulate the composition, diversity, and dysbiotic equilibrium of the oral microbiome. Analyzing longitudinal resting and stimulated saliva samples, the clinical trial tracked immediate and post-intervention microbial shifts. The interim molecular and taxonomic profiling demonstrated that both LLLT and TENS therapies not only enhanced objective salivary secretion rates but also induced critical ecological changes, suppressing acidogenic pathways and promoting a healthier microbial balance. Brief Abstract: This forward-thinking review article challenges conventional, reductionist dental microbiology by advocating a profound paradigm shift in managing dental caries: moving from pathogen eradication (“blame”) to ecological equilibrium (“balance”). Capitalizing on recent multi-omics, next-generation sequencing, and oral archaeome discoveries, the authors demonstrate that functional diversity and metabolic activity within the biofilm operate dynamically at the individual level, far beyond isolated taxonomies.
